Travel Insured International is the company that Untours recommends for trip cancellation/interruption insurance. I endorse them also, having insured with them for each of our many Untours. Our cancellation in 2006 was handled very well, both by Untours and by Travel Insured.
The car insurance Walter refers to is a Collision Damage Waiver provided by Travel Insured when you purchase their trip insurance. I have not used that feature, so I cannot say how convenient it is. Obviously, as Jerry says, the most convenient is to get collision coverage with your rental, but that coverage has always seemed quite pricey. But convenience/no headaches during your trip is worth something. Also, check with your US car insurance company - some provide insurance if you're driving a rental, but they may be limited to certain countries.
Finally, I agree with Jerry - don't bother with an International Drivers License. I got one through AAA many years ago before travelling to Brazil on business, but over many years of travel, have never had occasion to use it.

I'm not familiar with the Travel Insured International to which you refer, however, I do recommend that you include the collision coverage. This way there are no hassles should you have an accident. As for the International license, in several dozen trips I've never been asked for it and, after my first trip, never took one again. You'll not have any problem where you are going. I've driven in both countries during the past several years.
