We have used IDT Gobal Calling card for the past 4 years when we travel 
internationally (Italy - including Sicily, London, Brussels, Mexico).  It 
was recommended in a travel column in the LA Times and it has always worked 
effortlessly for us.  You register for the card and set up a mechnism to 
load increments of $25 to your card.  Access numbers have always worked for 
us.  There is a monthly fee of $1.99 deducted from the card as long as you 
have money loaded on it.  If the card is empty, there is no fee charged.  It 
just remains empty until you reload it for a trip.
